About the LTACH Leadership Institute
The organization behind the standard.
The LTACH Leadership Institute sets the professional standard for leading the care of the chronically critically ill: patients who survive catastrophic illness and then require weeks or months of hospital-level care, most often on a ventilator. The Long-Term Acute Care Hospital, the setting built around these patients, is where that standard takes its name and its evidence base.
The Institute's credential, the Certified LTACH Leadership Professional (CLLP™), is governed by an independent Credentialing Board with sole certifying authority. Its examination measures command of eight competency domains, from clinical operations and regulatory compliance to finance, workforce, and technology, spanning the full scope of the role.
Leadership hiring in this sector has always screened for tenure and setting, because those were the only qualities an employer could verify. The CLLP™ verifies what a leader knows, at the level of judgment a leader needs to read an unfamiliar situation, weigh competing obligations, and defend a decision.
The judgments that carry the greatest cost stay invisible until they go wrong, and the examination is built to reach them. Defining competent leadership for this population is work that belongs to the entire field. The Institute does it once, independently, for every operator, health system, and leader responsible for these patients.
The LTACH Leadership Institute exists to organize, advance, preserve, and disseminate the body of knowledge required to lead the care of the chronically critically ill. That knowledge exists today in fragments, held by individuals rather than by a discipline. The Institute assembles it, states it plainly, and keeps it current, anchored in the Long-Term Acute Care Hospitals where these patients are most concentrated and portable to every setting where they are found.
A future in which every leader responsible for the chronically critically ill arrives prepared for the most complex environment in American medicine, carrying a credential that reflects the weight of what they do and a standard defined by the leaders who have borne it.
Built from the bedside up.
Medicine named this patient four decades ago. Chronic critical illness describes the survivor of catastrophic illness who has passed the acute threat and has yet to recover: dependent on prolonged ventilation, complex wound care, and ICU-intensity intervention across weeks rather than days. The clinical science of this patient is deep and mature. The Institute brings the same rigor to the leadership of that care, with a standard built for this population, its regulatory environment, and its economics.
That standard begins with evidence. The CLLP™ competency framework rests on a formal Job Task Analysis of LTACH leadership practice, conducted with a panel of practicing leaders and professionals who work in and alongside LTACHs and facilitated by independent psychometricians. It documents what these leaders do, the decisions they own, and the competencies those decisions demand, organized into eight domains spanning the clinical, regulatory, financial, human, and technological dimensions of the role. The weight each domain carries comes from the field itself: a national blueprint survey, administered by independent psychometricians, asks practicing LTACH leaders across every function, along with the physicians, case managers, and contracted partners who work alongside them, to rate how often each task arises and how critical it is, and the final examination weights follow their answers.

Authority over that standard rests with the CLLP™ Credentialing Board, composed of active LTACH leaders and professionals with substantive, previously held LTACH executive roles, as well as experience in other chronically critically ill care environments. Collectively, they have answered for census, margin, and quality at the board table, carried federal and state survey exposure, run these hospitals and programs, and led the care of these patients at the bedside. They set the standard, validate the examination, and hold sole authority to award and revoke the credential, independent of the Institute's education and business activities.
The Institute's education is built by a team that includes MDs, PhDs, and EdDs, with expertise spanning medicine, nursing, and adult learning, alongside decades of LTACH operating experience and MBA-level command of healthcare finance and technology. Education prepares leaders for the work. The examination, governed entirely by the Credentialing Board, measures their command of it.

Practitioner-Designed
Everything we develop is designed with — not for — LTACH practitioners. Our Credentialing Board, our Job Task Analysis panels, and our curriculum advisory groups are composed of active LTACH leaders and recently retired LTACH executives who bring both current operational knowledge and the deeper perspective that comes from having seen the full arc of careers in this setting. Academic rigor without operational relevance is not rigor — it is theory. We build both.

Our founder holds a doctorate in adult education and higher learning. That is not a credential we display for prestige — it is the scientific foundation of how we design learning experiences. Adult professionals learn differently than students. Working healthcare professionals learn differently than full-time graduate students. Every CLLP curriculum decision reflects that andragogical understanding.
Institutionally Credible
Professional credentials derive their legitimacy from governance — not from government licenses or accreditation stamps. The CLLP Credentialing Board holds independent certifying authority grounded in its governance structure, its published standards, its psychometrically validated examination, and the professional standing of the practitioners who comprise it. This is how respected professional certification works. That authority exists now — it is structural, not aspirational, and it does not depend on any external body to activate it.
Governance

The CLLP Credentialing Board is the independent governing body responsible for certification standards, examination validity, and policy decisions. Board members, active LTACH practitioners and individuals with prior LTACH executive experience who now serve in leadership roles across other healthcare settings, receive a fixed annual honorarium that does not vary with certification volume, examination outcomes, or program revenue. The Board must maintain a majority of currently active practitioners at all times.
The Board's independence from the Institute's commercial operations is structural, not aspirational. Board members are prohibited from serving in a commercial role with the Institute while on the Board. Their authority over certification standards is absolute — and separate from the Institute's educational and business activities.

It sets the standards candidates must meet, validates the examination that measures competency against them, and awards the designation to those who qualify. That authority exists now — it is structural, not aspirational, and it does not depend on any external body to activate it.
Independent Certification Authority
The CLLP™ Credentialing Board serves as the independent certification authority for the program, exercising exclusive and non-delegable jurisdiction over all certification functions — the establishment of standards, the validation of competencies, the development and psychometric maintenance of the certification examination, and the issuance, suspension, and revocation of credentials.
Evidence-Based Competency Framework
The CLLP™ competency framework is grounded in a formally validated Job Task Analysis of LTACH leadership practice, ensuring that all domains, performance expectations, and assessment targets are grounded in documented evidence of professional practice. The certification standards are demonstrably linked to the actual knowledge, skills, and judgments required for competent performance in the field.
Psychometrically Defensible Examination
The examination system is developed, maintained, and periodically evaluated in accordance with professional testing and psychometric specifications. Development processes incorporate item-writing protocols, technical review, content validation, security controls, and ongoing psychometric monitoring — reflecting the quality-assurance expectations applied to high-stakes certification programs across the U.S. credentialing landscape.
